Having a complete history of your pet will enable our doctors to treat your pet more thoroughly. Also, please arrive at your appointment 15 minutes early to allow our receptionists time to enter your pets’ information into the computer. We look forward to your visit!
Should your pet be diagnosed and you are treating them for diabetes, it is very important to keep documentation of eating times, amount of insulin given, and how your pet responds through the day. This information is especially important if we have recently diagnosed your pet, or if we are trying to regulate your pet’s insulin levels. The following documentation log can be downloaded and printed at home for you to log your pet’s diabetes. Remember to bring this sheet in with you and your pet when you visit your veterinarian. He/She will use that information to properly adjust your pet’s insulin levels. Should you have any questions, feel free to write them down and ask your veterinarian at your pet’s appointment.

Proper dental care is very important in extending the health and happiness of your pet! We offer routine dental cleaning and extractions Monday through Friday. Plaque and tartar can cause bad breath and serious health concerns in your pet. By keeping your pet’s teeth clean on a regular basis, we can help extend the life of your pet as well as their teeth. The doctor will call you during the dental should we discover any new problems with your pet’s teeth. Should your pet require any extractions, we can perform those for you as well. Our dental consent form is available for you to look over at home before your pet’s dental procedure. Our surgery technicians will review this sheet again with you the morning of the procedure to discuss everything in detail. Should you have any questions, feel free to write them down and ask our technicians.
We offer the convenience of drop-off service for your pet at no additional fee. We realize that your schedule may not allow you time to wait during an appointment, even if your pet needs time-sensitive care. Also, your pet may require lab work or x-rays that may take longer than a routine appointment. Please click on the link to fill out the Drop-Off Sheet to make your check-in more efficient. Once your pet has been examined, one of our doctors will call you and keep you updated throughout the day.
Animals may suffer from seizures much like people. Causes for seizures may range from problems within the brain or outside the brain. If your pet suffers from seizures, use this worksheet to document the seizure activity and all pertinent information. If your pet is also on medication for seizures, make sure you document when you give the medication, as well as how much. This information will be helpful when your veterinarian is evaluating your pet. Should you have any questions, feel free to write them down and your veterinarian will be happy to answer them for you.
